UPDATE: As the health and welfare of our Signature HealthCARE residents is and always will be our top priority, we want to inform you that state and local officials of South Pasadena, Florida have made the decision to evacuate parts of South Pasadena, which includes our facility, Gulfport Rehabilitation Center.
Tuesday morning, our leadership and staff decided to begin safely transferring our beloved residents to two of our sister facilities:
Golfview HealthCARE Center

Signature HealthCARE teams are working tirelessly to preserve the safety and health of our residents and will continuously provide up-to-date information to Signature HealthCARE families.
Our Care Navigation Line at 1-844-SHC-CHAT or 1-844-742-2428 is available 24/7 and will be updated as new information becomes available. We ask that you please direct all phone calls to this number, and refrain from calling the facility, as all staff will be focused on transferring and welcoming new residents into the Middleburg and Golfview communities. Updates will also continue to be posted on the Gulfport Rehabilitation Center and Signature HealthCARE of Middleburg and Golfview Facebook pages and websites.
